Something similar to what caused the Potters Bar crash in 2002. This is going to leave a lot of egg on Network Rail's faces. But it is amazing that there weren't any other fatalities!

Even though the train's not at fault, I was wondering whether it would be prudent for the next rolling stock order to be a different class of train (Virgin Trains are wanting additional, new trains for their West Coast franchise). The reasoning being that if this, or future possible, incident had been due to a design flaw, it wouldn't result in lack of tracks due to a withdrawn fleet.
