Notice the pilot in that Asianic Airline is not being thrown under the bus, in fact the pilots union has come out and BLASTED the FAA for some of the comments they have made before any sort of real invest has been done.

MMA employees in Quebec are represented by the Syndicat des Métallos (a USW subsidiary) and the union has issued a statement in French blasting the company and its policies, not the least of which was one-man operation.

Lots of factors cited at the operational, company, and oversight levels. This is very much written as an "organizational failure" rather than simply focusing on an inadequate amount of handbrakes set and not tested.
